Immunization Information and Requirements

There are several immunizations that are required by the State of Wisconsin for students attending school. The school must have a record of your student's immunizations and the dates they were received. If this information is not on file a legal notice may be sent. If a student does not receive the required immunizations for religious, medical or other circumstances a waiver must be on file at the school.
